I understand that by completing this form I will be concurrently enrolled in the SDSU Pre-Nursing Major and BHSU Applied Health Sciences major, with SDSU as my home location (the institution at which I am pursuing my primary degree and at which I am applying for financial aid). Dr. Valeriah Big Eagle will be my primary advisor.
